BackgroundUpper limb impairment is common after stroke, and many will not regain full upper limb function. Different technologies based on surface electromyography (sEMG) have been used in stroke rehabilitation, but there is no collated evidence on the different sEMG-driven interventions and their effect on upper limb function in people with stroke.AimSynthesize existing evidence and perform a meta-analysis on the effect of different types of sEMG-driven interventions on upper limb function in people with stroke.MethodsPubMed, SCOPUS, and PEDro databases were systematically searched (...) for eligible randomized clinical trials that utilize sEMG-driven interventions to improve upper limb function assessed by Fugl-Meyer Assessment (FMA-UE) in stroke. The PEDro scale was used to evaluate the methodological quality and the risk of bias of the included studies. In addition, a meta-analysis utilizing a random effect model was performed for studies comparing sEMG interventions to non-sEMG interventions and for studies comparing different sEMG interventions protocols.ResultsTwenty-four studies comprising 808 participants were included in this review. The methodological quality was good to fair. The meta-analysis showed no differences in the total effect, assessed by total FMA-UE score, comparing sEMG interventions to non-sEMG interventions (14 studies, 509 participants, SMD 0.14, P 0.37, 95% CI –0.18 to 0.46, I255%). Similarly, no difference in the overall effect was found for the meta-analysis comparing different types of sEMG interventions (7 studies, 213 participants, SMD 0.42, P 0.23, 95% CI –0.34 to 1.18, I273%). Twenty out of the twenty-four studies, including participants with varying impairment levels at all stages of stroke recovery, reported statistically significant improvements in upper limb function at post-sEMG intervention compared to baseline.ConclusionThis review and meta-analysis could not discern the effect of sEMG in comparison to a non-sEMG intervention or the most effective type of sEMG intervention for improving upper limb function in stroke populations. Current evidence suggests that sEMG is a promising tool to further improve functional recovery, but randomized clinical trials with larger sample sizes are needed to verify whether the effect on upper extremity function of a specific sEMG intervention is superior compared to other non-sEMG or other type of sEMG interventions. IntroductionThe assessments of the motor symptoms in Parkinson’s disease are usually limited to clinical rating scales, and it depends on the clinician’s experience. This study aims to propose a machine learning technique algorithm using the variables from upper and lower limbs, to classify people with PD from healthy people, using data from a portable low-cost device. And can be used to support the diagnosis and follow-up of patients in developing countries and remote areas.MethodsWe used Kinect®eMotion system to capture the spatiotemporal (...) gait data from 30 patients with PD and 30 healthy age-matched controls in three walking trials. First, a correlation matrix was made using the variables of upper and lower limbs. After this, we applied a backward feature selection model using R and Python to determine the most relevant variables. Three further analyses were done using variables selected from backward feature selection model, movement disorders specialist, and all the variables from the dataset. We ran seven machine learning models for each model. Dataset was divided 80% for algorithm training and 20% for evaluation. Finally, a causal inference model using the DoWhy library was performed on Dataset B due to its accuracy and simplicity.ResultsThe Random Forest model is the most accurate for all three variable Datasets followed by the support vector machine. The CIM shows a relation between leg variables and the arms swing asymmetry and a proportional relationship between ASA and the diagnosis of PD with a robust estimator.ConclusionsMachine learning techniques based on objective measures using portable low-cost devices are useful and accurate to classify patients with Parkinson’s disease. This method can be used to evaluate patients remotely and help clinicians make decisions regarding follow-up and treatment. (shrink)

Simultaneous discovery in science has been a subject of close historical investigations, not only for assessing claims of priority, which occasionally generate controversy rather than consensus, but also for understanding the cultural and intellectual context of the time. Thomas S. Kuhn is a pioneer in the contextual study of simultaneous discovery, and his paper on the formulation of the first law of thermodynamics has already become a classic.

Careerism refers to an individual’s propensity to achieve their personal and career goals through nonperformance-based activities. We investigated the role of several dispositional predictors of careerism, including Five-factor model personality traits, primary psychopathy, and exchange ideology. Based on data from 131 respondents, as expected, we observed that emotional stability was negatively correlated with careerism. Primary psychopathy and exchange ideology explained additional variance in careerism after accounting for FFM traits. Relative importance analyses indicated that psychopathy and exchange ideology were equally important (...) in predicting careerism. We highlight the need for future research efforts investigating the combined effects of contextual factors—particularly, human resource practices—and individual differences to understand careerism in the workplace. This is a detailed commentary on Hume's first Inquiry. Flew argues, rightly, that it should not be treated simply as a weakened abridgement of part of the Treatise. He gives a great deal of the historical context in an interesting and helpful way, but he is primarily concerned to lay out and to assess Hume's arguments. Inevitably much of the book covers quite familiar ground, but in discussing Hume's arguments on miracles and on religion generally, Flew has a number of (...) new and suggestive points to make. The book as a whole will be useful for the student beginning the study of Hume, and Flew's criticisms, as well as his frequent review of other criticisms, should prove helpful to the advanced student. --J. After a chapter on the theory of the concrete universal, Milne discusses the moral and political views of Bradley, Bosanquet, Green, and Royce. Milne's view is that the social philosophy of Idealism is permanently valuable, the metaphysics not. The work of Bradley and Bosanquet, he argues, is weakened by unnoticed ambiguities in their conception of the concrete universal; Green's work, though more consistent, involves a fundamental error in the theory of knowledge; and there is doubt as to the consistency of (...) Royce's metaphysics with his ethics. We can develop a humanistic Idealism which will be free of the metaphysics of the Absolute and the conservative tendencies of that view, Milne says, and his expositions and criticisms of earlier views are part of his effort to do this. The theory of a scale of standards of rationality in activity, on which Milne rests his reconstruction, needs to be more clearly developed, but it is useful to have so sympathetic a discussion of this general point of view.--J. Passmore examines a number of kinds of argument frequently used by philosophers, in an attempt to find out whether there is any kind of reasoning which is especially appropriate for philosophy. He discusses the ways in which philosophers have used deduction, induction, reminders about obvious facts, infinite regress arguments, paradigm case arguments, claims that certain views are self-refuting, and accusations of meaninglessness. Numerous illustrations of these moves in argument, drawn from philosophers from Plato to Popper, help to make this an (...) interesting essay in the philosophy of philosophy.--J. B. S. (shrink)

Chapters on the nature and beginnings of tragedy introduce a survey of Greek, Elizabethan, Neo-Classical, and Modern tragedy. The theoretical portions attempt to remedy Aristotle's "rather strange neglect of the philosophical and religious implications of tragedy," or "the tragic sense of life." Rather than a fresh theory of tragedy, the book presents a critical synthesis of the leading theories of cultural anthropology and psychology; of the work of the modern critics of archetypal forms and ideas ; and the "heroic humanism" (...) of critics like Cedric Whitman. The central sections have the vivid sweep of Muller's The Uses of the Past and give much of its indispensable background material.--R. B. S. (shrink)

Drawing from the Boulding''s (1956) framework for general systems theory, the need to employ richer paradigm in the study of organizations (Pondy and Mitroff, 1979) is reiterated. It is argued that a better understanding of organizational ethical behavior is contingent upon viewing organizations as symbol processing systems of shared language and meanings. Further, it is proposed that organizations, like individuals, develop into collectivities of shared cognitions and rationale, over a period of time. The study adapts Kohlberg''s (1983) model of moral (...) development to examine if organizations can be viewed as passing through different stages of moral development depending on the type of moral reasoning employed to explain their behaviors in the face of ethical crises. Several cases raising questions about the ethics of corporate behavior were researched. An instrument entitled Organization Response Analysis was constructed using statements made by various spokespersons representing five of these organizations. The instrument was administered to 246 graduate and undergraduate students of business (N = 246). Results indicated a great degree of concurrence among respondents of differing genders, levels of education and work experience in determining the stages of moral development of organizations. Limitations of the current study and implications for future research and practice are discussed. (shrink)

This volume of the Great American Thinkers series purports to let Thoreau speak for himself, primarily through passages quoted from his journals. Originally published in fourteen volumes, the journals represent over twenty years of Thoreau's life, and are the background and, in some cases, the original form of works more polished and more widely known. Murray has aptly considered Thoreau's wide range of thought and comment under several main headings, such as "Primacy or Purpose," "Society as Burden," and "Freedom and (...) Simplicity." In this arrangement, many of Thoreau's specific and often curt observations can be seen in a more general context, with Thoreau himself providing some of the keys to the transition.--G. B. S. C. In the title essay the late "spiritualist" philosopher and professor at the Collège de France, holds that suffering is in itself a morally neutral and necessary ingredient of the human condition, dependent for its value on the freely-taken attitude of the individual will. Especially noteworthy are Lavelle's non-technical and limpid descriptions of the various attitudes, negative and positive, through which suffering becomes either a debilitating or a deepening experience. More clearly Christian in perspective, the other essay, "Tous les Etres Séparés (...) et Unis" affirms that only through an extreme solitude that discovers our common source in God can a communion with others, which is neither superficial nor destructive of the separateness of morally responsible individuals, be achieved.--B.
